How to Claim

Submit a photo of the valid receipt via email to B+L at ULTRA@bausch.com, along with NAME, TEL, ADDRESS, EMAIL ADDRESS and NRIC/FIN* (last 4 characters).

*Passport no. (last 4 characters) may be used as an alternative to NRIC/FIN, for claim submission.

Closing date for claim submission: 30 April 2022

B+L reserves the rights to amend any Terms and Conditions without prior notice. Submission of claim implies automatic acceptance of all terms and conditions of the Promotion. In the event of dispute, B+L’s decision is final.

Terms & Conditions

• •   ULTRA ONE DAY Gift-with-Purchase Promotion is open to all residents of Singapore.
• •   Promotion is valid with purchase of 4 boxes of ULTRA ONE DAY in a single receipt.
• •   Applicable for purchases made between 1 January to 31 March 2022 (both dates inclusive) at optical shops located in Singapore
•      only.
• •   All claim submissions must be emailed to B+L before/on the closing date 30 April 2022, to be eligible for the Gift.
• •   Eligibility of the claim to the Gift will expire after closing date on 30 April 2022.
• •   Submission of Claim for Gift can be made only via email to B+L (at ULTRA@bausch.com) with photo of a valid receipt, Full name,
•      Tel, Address, Email and NRIC/FIN (last 4 characters). B+L reserves
•      the right to invalidate submission that is incomplete.
•     *Passport no. (last 4 characters) may be used as an alternative to NRIC/FIN for Claim submission.
• •   Gift will be shipped out within 3 weeks from claim submission date.
• •   Valid for local addressee in SG only.
• •   Only local addresses within Singapore are eligible for GWP Promotion.
• •   Gift is strictly non-exchangeable and non-replaceable for credit, cash or kinds in all cases, whether in whole or in part.
• •   Claimant is subjected to verification of eligibility and compliance with all terms and conditions set forth.
• •   B+L reserves the rights to verify and invalidate duplicated/multiple claim submission with the same email address/contact number
•      &/or receipt.
• •   Should there be dispute as to the identity of a claimant, B+L reserves the right, in its sole discretion, to determine the identity of the
•      claimant – who is eligible for the Gift under the Promotion.
• •   The gift will be sent out within Three (3) weeks from claim submission date. You may email to B+L (at ULTRA@bausch.com) if you
•      do not receive the gift email after 21 days from claim submission
•      date.
• •   B+L is deemed to have fulfilled its obligation for GWP Promotion once the Gift has been sent to the 3rd party logistics company.
•      B+L will not be hold accountable for any missing parcel or due to
•      incorrect address provided at the point of claim submission.
• •   The Gift will be sent out through a third-party logistics company and claimant will not be able to select any preferred time slot for
•      delivery.
• •   Consumer will receive a notification of delivery and tracking ID from the third-party logistics company on day of shipping.
• •   Decisions of the organizer are final, and no correspondences will be entertained.
• •   B+L may, at any time and without notice in its sole and absolute discretion, substitute any Gift with any other item of a similar value
•      without giving notice.
• •   Any and all personal data submitted may be collected, used and/or disclosed to B+L and its assigned agencies for various
•      purposes, including to communicate with the claimant for purposes related to
•      the Promotion, and such other purposes B+L may reasonably deem appropriate, or in relation to which the claimant has otherwise
•      provided B+L with consent.
• •   The Company will only retain Personal Data for as long as necessary for the fulfilment of the purpose for which it was collected or
•      to which you have given your consent, except where otherwise
•      provided by law.
• •   If for any reason any aspect of promotion is not capable of running as planned, including by reason of computer virus, technical
•      failure, or any cause beyond the control of the Company, the Company
•      may in its sole discretion cancel, terminate, modify suspend the promotion.
